Exkurs: Imprinting und Original Antigenic Sin (OAS)
Imprinting: Das Immunsystem bildet gegen Antigene (z.B. Virenbestandteile), mit denen es zum ersten Mal in Kontakt kommt, Gedächtniszellen aus und wird auf diese “geprägt”. Das ist evolutionär sinnvoll, weil die Gedächtniszellen bei erneutem Kontakt (Infektion) schnell spezifische Antikörper produzieren können. Die Gedächtniszellen reifen zudem im Verlauf der Zeit in den Keimzentren, z.B. Lymphknoten durch die Affinitätsreifung. Durch die somatische Hypermutation werden ständig neue Gedächtniszellen prduziert und jeweils diejenigen, deren Antikörper das feindliche Antigen am besten neutralisieren, selektiert.
Gefahr: Bei der nächsten Virusinfektion bekämpft das Immunsystem die bekannten Virusbestandteile besser und effektiver, das geht aber zulasten einer effektiven Reaktion gegen neue Mutanten/Virusbestandteile ( = OAS). Für SARS-CoV2 bisher noch nicht nachgewiesen (Chemaitelly et al. 12.10.22).

Risikoabwägung für junge Frauen (20-29) bei geringer Inzidenz (20 pro 100000):
Exkurs: Wirksamkeit von Impfstoffen (von PhD Nathalie E. Dean)
Die Impfstoffwirksamkeit (VE) misst die relative Verringerung der Infektion/Krankheit im geimpften Zweig gegenüber dem ungeimpften Zweig. Ein perfekter Impfstoff würde das Risiko komplett eliminieren, VE wäre dann 1 oder 100%. Das kann aus dem Risikoverhältnis, der Inzidenzfallrate oder der Risikoquotient (hazard ratio) berechnet werden.
Eine VE von 50% bedeutet grob, dass ein um 50% verringertes Risiko hat, krank zu werden gegenüber einer sonst ähnliche, aber ungeimpften Person. Oder man hat eine 50%-Chance, krank zu werden, wenn man so viel infektiösem Virus ausgesetzt wurde, dass es ausgereicht hätte, eine ungeimpfte Person krank zu machen.
Dazu unterscheidet man noch die Wirksamkeit, eine Infektion zu verhindern (sterilisierende Immunität), die Krankheit zu verhindern und einen schweren Verlauf zu verhindern. Die meisten Phase III-Studien messen zunächst die Wirksamkeit, die Erkrankung zu verhindern und danach Infektion und schwere Verläufe. Die Infektion zu verhindern ist die größte Herausforderung, ebenso ein Impfstoff, der Krankheit und schweren Verlauf verhindert. Doch können wir Impfstoffe haben, wo die Leute immer noch infiziert werden, aber ihre Erkrankung gemildert wird. Die Fähigkeit eines Impfstoffs, die Infektiösität gegenüber anderen zu verringern, ist als indirekter Schutz bekannt und mit Herdenimmunität verwandt. Wenn ich mich nicht infizieren kannst, kann ich Dich nicht infizieren. Haushaltsstudien sind hier sehr nützlich, wo man die Haushaltskontakte von infizierten geimpften und ungeimpften Individuen überwacht, und vergleicht, wie häufig sie sich infizieren.
Letzter Punkt: Wirksamkeit (efficacy) versus Leistung (effectiveness)
Wir möchten den Begriff efficacy für Schätzungen von randomisierten Studien reservieren, wo jeder den Impfstoff absichtlich erhält (korrekte Kühlkette, keine fehlenden Dosen). Wir unterscheiden diese idealisierte Messgröße von praxisorientierter effectiveness.
